What is the factual accuracy process and how do I complete it?

Following the site assessment, the report will be reviewed by the QA assessor, who have not been involved in the assessment so far. After the QA has reviewed the report, it will then be sent to the service for a factual accuracy (FA) check.

The aim of the FA review is to allow the service to make any comments where they believe the report is incorrect or does not reflect the service. If anything is incorrect, please click 'No' and add a comment outlining the error. This will then be reviewed by the lead assessor who can make any necessary changes to the report/discuss this further with the service in response.

If you are happy with the report then you will only need to scroll down and click submit at the bottom of the FA check. Once the FA check has been completed the report will be signed off by the QPIDS accreditation programme chair and sent to your service.
